Well, it wasn't answered exactly where Penny was born, but the fact that she wasn't born on the island should be enough to change this mystery to answered (because we don't really care where it was if it wasn't on the island :P)

ANSWERED in 5.12, Dead is dead. Desmond beat him up when Ben was "trying" to kill Penny.

In "The Constant", Penny tells Desmond that she knows about the Island, and has been looking for him for three years. So, the "IT" is confirmed to be the Island.

As revealed by George Minkowski in "The Constant", Naomi's freighter was receiving regular calls from Naomi, looking for Desmond (Minkowski knew Desmond's name from Penny's calls).

Answered by Damon and Carlton in this EW article. Penny had autodialers set up around her house that were constantly seeking a signal. When Charlie cut off the jamming signal, they automatically dialed in.
